What to Bring: Immigration Medical Exam Appointment

Make your medical appointment at Barrie Immigration Medical Centre hassle-free by knowing what to bring to your immigration medical exam such as IDs, documents, and essential health records. Here’s a simple checklist to help you prepare:

Government Issued Photo ID (must bring 1 of the following)

  • Original valid passport (preferred) or a certified copy from a Canadian Visa Office 
  • National ID card 
  • Canadian driver’s license 
  • IFH Refugee Protection Claimant document (if you are applying as a refugee/asylum claimant, this is a valid photo ID)

Immigration Documents

IRCC Medical form (IMM1017): If you have received the IRCC medical form IMM1017, please bring it with you. Upfront medicals do not receive these forms.

If you are a refugee/asylum claimant, please bring one of the following:

  • Refugee Protection Claimant Document
  • Acknowledgement of Claim and Notice to Return for Interview
  • Refugee travel document
  • The Interim Federal Health Certificate

Medical Information

  • Eyeglasses or contact lenses: If you use eyeglasses or contacts, please bring them with you.
  • Medical history documents: If you have any medical reports or test results for any previous or existing medical conditions, please bring them with you.
  • List of medications: Make a list of all prescribed medications along with their dosage and bring them for your appointment.
